If you are over 50 and you have noticed that straight lines are starting to look wavy, or that reading has become harder even with the right glasses, there is a condition you need to know about. Age-related macular degeneration — known as AMD — is the leading cause of severe vision loss in people over 50 in developed countries. It affects the macula, the small but critical area at the centre of the retina that is responsible for the detailed, sharp vision you use every day — for reading, driving, recognising faces, and seeing the world in colour and focus.
AMD does not cause total blindness. Peripheral vision is preserved. But the loss of central vision can be profoundly disabling. This article explains the two forms of AMD, the warning signs to watch for, and the treatments that can make a meaningful difference — especially when the condition is caught early.
Dry AMD: The Slow, Gradual Form
Approximately 85% to 90% of all macular degeneration is the dry form, also called atrophic AMD. In dry AMD, small yellow deposits called drusen accumulate beneath the retina. Over time, these deposits contribute to a gradual thinning and breakdown of the macular cells. Vision loss is typically slow and may progress over many years.
There is currently no cure for dry AMD. However, a specific combination of nutritional supplements — known as the AREDS2 formula, which includes vitamins C and E, zinc, copper, lutein, and zeaxanthin — has been shown to slow the progression from intermediate to advanced dry AMD. Lifestyle modifications also help: not smoking, eating a diet rich in leafy greens and omega-3 fatty acids, managing blood pressure, and protecting the eyes from excessive UV exposure.
Perhaps most importantly, patients with dry AMD need regular monitoring, because a small but significant percentage of cases will convert to the more aggressive wet form.
Wet AMD: The Urgent, Treatable Form
Wet AMD — also known as neovascular AMD — accounts for only 10% to 15% of cases but is responsible for approximately 90% of severe vision loss from macular degeneration. In wet AMD, abnormal blood vessels grow beneath the retina. These vessels are fragile and leaky. They bleed and leak fluid into the macula, causing rapid distortion and loss of central vision.
The critical difference between wet and dry AMD is urgency. Wet AMD can cause significant damage in a matter of weeks if untreated. But it is also the form that responds best to treatment — if caught early.
The hallmark early warning sign of wet AMD is metamorphopsia — a distortion of vision where straight lines appear wavy, bent, or broken. If you notice this, seek an urgent appointment with an ophthalmologist. Do not wait.
The Warning Signs to Watch For
Macular degeneration often begins with subtle changes that are easy to dismiss. The symptoms to be aware of include:
- Blurring or distortion of central vision
- Straight lines appearing wavy or crooked (the most important warning sign)
- A dark or blank patch appearing in the centre of your field of vision
- Difficulty reading fine print even with glasses
- Colours appearing less vivid or washed out
- Needing brighter light for close-up tasks like reading or sewing
A simple home monitoring tool is the Amsler grid — a small square grid of straight lines with a dot in the centre. Viewed one eye at a time, any waviness, blurring, or missing areas on the grid can indicate macular changes and should prompt an ophthalmology appointment.
How AMD Is Diagnosed
A specialist ophthalmologist diagnoses and monitors AMD using several complementary tools:
- A dilated fundus examination allows direct visualisation of the macula, drusen deposits, and any signs of bleeding or fluid.
- Optical coherence tomography (OCT) is the gold-standard imaging tool — it produces cross-sectional images of the retina with extraordinary detail, revealing fluid, thickening, or structural changes invisible to the naked eye.
- Fluorescein angiography involves injecting a dye to photograph blood flow through the retinal vessels, identifying any abnormal or leaking vessels characteristic of wet AMD.
- OCT angiography (OCTA) is a newer, non-invasive technology that images the retinal blood vessels without the need for dye injection.
Together, these tests provide a comprehensive picture of the type, stage, and activity of the disease — guiding treatment decisions with precision.
Treatment for Wet AMD
The treatment that has transformed outcomes for wet AMD is anti-VEGF injection therapy. VEGF (vascular endothelial growth factor) is the protein that drives abnormal blood vessel growth. Anti-VEGF medications — such as ranibizumab and aflibercept — are injected directly into the eye under local anaesthetic. The procedure takes only a few minutes and is far less uncomfortable than most patients expect.
These injections do not cure wet AMD, but they can stop the leaking, shrink the abnormal vessels, and in many cases stabilise or even improve vision. Treatment typically begins with a series of monthly injections, followed by a personalised schedule based on how the eye responds. Ongoing monitoring with OCT imaging is essential to guide the timing of each treatment.
The single most important factor in treatment success is timing. Patients who begin anti-VEGF therapy early — before significant scarring has occurred — consistently achieve better visual outcomes.
Living Well With AMD
A diagnosis of macular degeneration is not a sentence of total blindness. Peripheral vision is preserved, and with appropriate treatment and monitoring, many patients maintain functional central vision for years or even decades. Low-vision aids, magnification devices, adjusted lighting, and occupational therapy support can all help maintain independence and quality of life.
