Ask most people to name a serious eye condition and they will say cataracts or glaucoma. Almost nobody mentions macular degeneration. Yet it is the leading cause of severe, permanent vision loss in people over 50 in the developed world — and its impact on daily life is devastating in a way that statistics alone cannot capture.
Macular degeneration does not take all of your sight. It takes the centre. The part you use to read a grandchild’s face. To follow a recipe. To drive a familiar road. To thread a needle, read a message, or recognise someone across the room. The peripheral vision remains, but the life lived through the middle of your gaze quietly disappears.
This article is for anyone over 50 — and for anyone who loves someone over 50. It is about a condition that deserves far more attention than it gets, and about what can be done when it is found early enough to make a difference.
What Macular Degeneration Actually Is
The macula is a tiny area at the very centre of the retina — the light-sensitive tissue lining the back of the eye. Though it measures only about 5 millimetres across, the macula is responsible for all of your detailed, sharp, central vision. It is what allows you to read, drive, see colour accurately, and perceive fine detail.
In age-related macular degeneration — commonly called AMD — the cells in the macula progressively deteriorate. The result is a gradual or, in some cases, sudden loss of central vision while the edges of your visual field remain intact. Patients often describe it as trying to see through a smudge, a blur, or a dark patch in the centre of everything they look at.
Two Forms, Two Very Different Stories
Dry AMD: The Slow Progression
Roughly 85% to 90% of cases are the dry form. It begins with small yellow deposits called drusen beneath the retina. Over months and years, these contribute to a slow thinning and breakdown of the macular tissue. Vision loss is gradual and may progress over many years.
There is no cure for dry AMD, but the AREDS2 supplement formula has been clinically shown to slow the progression from intermediate to late-stage disease. Lifestyle factors matter too: stopping smoking, eating a diet rich in dark leafy greens and oily fish, maintaining healthy blood pressure, and protecting the eyes from UV exposure.
Perhaps most critically, patients with dry AMD need regular monitoring, because a portion of cases will convert to the more aggressive wet form — and catching that conversion early changes everything.
Wet AMD: The Urgent Threat
Wet AMD accounts for only 10% to 15% of all cases but is responsible for approximately 90% of the severe vision loss. Abnormal blood vessels grow beneath the retina. These vessels are fragile and prone to leaking blood and fluid directly into the macular tissue. The damage can be rapid — significant central vision loss can occur in a matter of weeks.
This is the form that demands urgency. But it is also the form that responds most powerfully to treatment — if caught before permanent scarring has set in.
The Signs That Something Has Changed
Macular degeneration can be subtle in its early stages. The symptoms to watch for include:
- A gradual blurring of central vision that glasses do not correct
- Straight lines appearing distorted or wavy
- A dark or empty area developing in the middle of your vision
- Difficulty reading even in good light
- Colours that seem less vivid
- Needing significantly brighter light for close-up tasks
A simple home monitoring tool is the Amsler grid — a small square grid of straight lines with a central dot. Cover one eye at a time, focus on the dot, and observe whether the lines appear straight and complete. Any waviness, blurring, or missing areas warrant an appointment.
How It Is Diagnosed and Monitored
A specialist ophthalmologist diagnoses AMD using several complementary tools:
- Dilated fundus examination — allows direct visualisation of the macula.
- OCT (optical coherence tomography) — the gold-standard imaging tool, producing cross-sectional images of the retina with extraordinary detail.
- Fluorescein angiography — photographs blood flow through the retinal vessels, highlighting leakage.
- OCTA — newer technology that images the vasculature without dye injection.
Treatment That Makes a Real Difference
For wet AMD, anti-VEGF injection therapy has transformed outcomes. These medications, injected into the eye under local anaesthetic, block the protein driving abnormal vessel growth. They can stop leaking, shrink vessels, and stabilise or even improve vision. Treatment begins with monthly injections, then transitions to a personalised schedule guided by OCT monitoring. The single most important factor in success is timing — patients who begin early achieve meaningfully better outcomes.
